Priority Health Coaching Program – Cigna

If you’re living with Asthma, COPD, Depression, Diabetes, Heart Disease, or other long-term conditions, Cigna’s trained nurses, nutritionists, and health coaches can help you with:
- Medication guidance, education, and symptom tracking
- Recognizing early warning signs and know when to see a doctor

Diabetes Care Management Program – Transform Care

Designed to make managing diabetes easier and more affordable, this program is offered to members at no cost.
- A connected glucometer and unlimited test strips and lancets.

Covered Therapies and Treatments in the CDHP
Under the Consumer Driven Health Plan, you can receive broad therapy support for medically necessary treatments.
Included Treatments:
- Home IV and antibiotic therapy
- Respiratory/inhalation therapy
- Growth hormone therapy
- Chemotherapy and radiation
- Dialysis (hemodialysis and peritoneal)
- Cardiac rehab (Phases I & II)
- Pulmonary rehab
Cost Breakdown:
- In-Network: 20% of the Plan allowance
- Out-of-Network: 50% of the Plan allowance + any difference between our allowance and the charge amount.

Specialty medications required for chronic condition management are fully supported under the PSHB CDHP.

The NALC CDHP offers personalized support for 19 complex conditions through Accordant Health Management, including ALS, Crohn’s Disease, Multiple Sclerosis, Lupus, Parkinson’s, Rheumatoid Arthritis, and more.
Through Cigna’s Priority Health Coaching program, you’ll receive one-on-one coaching from nurses, nutritionists, and health advocates to manage symptoms, stay on track with treatment, and improve daily habits.
All specialty medications require prior approval and must be filled through CVS Specialty. Support includes home delivery, clinical guidance, and access to copay assistance programs, if eligible.
