CDHP Deductible and Out-of-Pocket Maximums

Annual Deductible (In-Network): $2,000/person - $4,000/family

Annual Deductible (Out-of-Network): $4,000/person - $8,000/family

Annual Out-of-Pocket Maximum (In-Network): $6,600/person - $12,000/family

Annual Out-of-Pocket Maximum (Out-of-Network): $12,000/person - $24,000/family

Important Notice: When you get emergency care, use an out-of-network air ambulance provider, or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing. “Surprise billing” is an unexpected balance bill as defined by a new federal law called the No Surprises Act.
